Case Manager LOCATION:

Summary:
This position requires staff to perform several tasks to prevent Veteran and family member(s) suicide.
These duties include working directly with the Veteran, family members, health care workers, and community agencies/ public and private.
The Case Manager (CM) will meet with the Veteran in the office, home visits, or at a location, the Veteran is staying.
They will work directly with the facility the Veteran is transitioning out of and assist with the discharge plan.
The CM will advocate for the Veteran and assist with appointments and transportation.
The CM will develop an after care plan with the Veteran, family members and professionals working with the Veteran.
Case Managers will attend multidisciplinary meetings involving the Veteran.
The CM will document (case notes) all activities involving the Veteran and family member(s) the CM was involved in.
The CM will perform outreach efforts, and assist with providing classes in Veteran Suicide prevention to the community while always promoting zero suicides.

Qualifications:
Bachelor's Degree in the Social Sciences and 5 years' experience working with Veterans.
Knowledge of community resources Reliable transportation.
Experience in developing care plans.
Strong verbal and written communication skills.
Critical thinking and problem solving skills to determine best care and securing resources.
Able to adjust care plan if positive results are not happening.
Time management and organized to manage different cases at the same time.
Maintain regular attendance.
Able to perform work activities in a high pressure and time sensitive environment.
Responsibilities:
Recruit, build rapport, and retain Veteran involvement in suicide prevention activities.
Perform VA approved assessments.
Develop individual care plan.
Document care activities (case notes).
Coordinate with VA facilities, community based organizations and private practice health providers in addressing Veteran needs.
Monitor Veteran progress and oversee Veteran case file.
Provide timely reports and work with QA staff.
Along with Project Director serve as liaison with community resources.
Perform outreach in the community.
Work with Communications Specialist to provide prevention training to Veteran and family members, community residents and organizations, schools, businesses especially those who sell weapons.
Work with Veteran and family members in applying and securing public benefits.
Other duties as required.

Good listener.
Ability to manage time effectively.
Able to coordinate the provision of Veteran services Ability to work as part of a team.
