Responsibilities:
Serves as a liaison between the client, referral sources and community resources.
Receives documentation from providers and follows up with clients regarding their treatment plan and referrals.
Sets up and attends meetings with providers and clients as needed.
(Child Family Teams (CFT's) Informs all clients involved on the processes that are being used to treat the client.
Inform and educate the client and family members involved in ongoing treatment plans for the clients and provide the client with written information regarding their treatment plan and other available resources.
Maintain complete, accurate, and timely records on each client using Theranest EMR or paper documents (depending on members funding source).
Maintain client confidentiality in treatment plans and reporting as appropriate.
Understands and abides by HIPAA guidelines.
Delivers behavioral health education services.
Provide materials such as audiovisual aids, articles, brochures, fact sheets and public service announcements promoting behavioral health topics.
Collects and analyzes data from various sources to evaluate programs and determine the need for additional educational programs or communications.
Develops curriculum for community education and offer in-service sessions for THJ staff and outside organizations.
Works with community contacts to promote behavioral health programs, teach individual and group classes, and work with groups to solve community health issues.
May be asked to provide input on grant proposals and report on program/clientele progress.
Performs other duties as assigned.
Required Skills or Abilities 1)Excellent written and verbal communication skills and ability to communicate complex information 2)Ability to work with diverse population of members and staff members 3)Excellent computer skills 4) Meet the documentation standards of 48 hours 5)The ability to communicate effectively and compassionately with people who have mental health issues.
6) Must be bilingual in English and Spanish i.

must read, write and speak fluently.
Required Knowledge, Experience or Licensure/Registration 1)Actively working towards a bachelor's degree or currently has a bachelors in social work, psychology or counseling or equivalent experience require.
2)Experience with MS Office products required; previous EMR experience preferred.
3)Reliable transportation is required for travel between the probation, Health Homes, providers and other agencies.
